Beverly Hills and the neighboring city of West Hollywood are together entirely surrounded by the city of Los Angeles. The area's so-called "Golden Triangle" of wealthy neighborhoods is formed by Beverly Hills and the Los Angeles neighborhoods of Bel-Air and Holmby Hills.&lt;br /&gt;&lt;br /&gt;Beverly Hills is bordered on the north by Bel-Air and the Santa Monica Mountains, on the east by West Hollywood and the Fairfax District of Los Angeles, on the south by Los Angeles' Beverlywood neighborhood and on the west by Holmby Hills as well as Westwood Village and Century City, which are also Los Angeles neighborhoods and not separate cities. It was the Spanish explorers and missionaries who transformed California from Indian territory to ranching and farming country. &lt;br /&gt;&lt;br /&gt;In 1854, El Rancho was sold for $4,000 to Major Henry Hancock and Benjamin Davis Wilson who used it to farm wheat, ranch cattle, produce wool and pump oil. Today BOTOX® Cosmetic injections have become a popular method for softening forehead lines, crow's feet, and frown lines.&lt;br /&gt;&lt;br /&gt;There are a couple of factors that cause the skin to wrinkle in these areas - the progressive loss of elasticity due to aging and the stretching and contraction of the muscles in the upper portion of the face causing a folding of the skin. BOTOX® Cosmetic is injected into the muscles of the upper face, weakening or paralyzing the muscle, which lessens the lines of facial expression.&lt;br /&gt;&lt;br /&gt;It can take 24 hours, up to 10 days for results to appear and they will last anywhere from three to six months.&lt;br /&gt;&lt;br /&gt;The sites that are most commonly treated by BOTOX® Cosmetic include the forehead, which would minimize the transverse wrinkle lines, the crow's feet area, and the glabellar complex (the area between the eyebrows containing the corrugator and procerus muscles).&lt;br /&gt;&lt;br /&gt;Procedure: Although there are no drugs that an otherwise healthy patient would be taking that would interact with BOTOX® Cosmetic, there are some things you should avoid before treatment. You should avoid taking anti-inflammatory medication, Vitamin E and aspirin for two weeks prior to treatment. Each of these medications may cause bruising when the BOTOX® Cosmetic is administered.&lt;br /&gt;&lt;br /&gt;The entire procedure takes approximately 10 minutes, and is given through a very fine gauge needle, minimizing the discomfort from the injection. If patients have a very low pain threshold, it is recommended that they take Tylenol ® or other medication prior to the injection. Topical anesthesia with Emla ® or another anesthetic may be used. After the treatment, we will ice the area to reduce the swelling and discomfort. After you have received the BOTOX® Cosmetic injections, wait four hours before lying down, do not massage the treated muscles as this can cause the botulinum toxin to spread to the muscles around the eyes. Exercising the muscles every 15 minutes for one hour after the treatment probably increases the effect.&lt;br /&gt;&lt;br /&gt;BOTOX® Cosmetic treatments can be repeated every four to six months and to date, no long term side effects have been detected. Patients will require less frequent treatments as they break the habit of contracting frown lines, thereby causing atrophy of the underlying muscle. &lt;br /&gt;&lt;br /&gt;Complications from BOTOX® Cosmetic: There are a few possible side effects from the BOTOX® Cosmetic injections but they are rare and always reversible. Sometimes an injection is not effective and a repeat is necessary. In this manufacturer-supported, double-blind, randomized, parallel-group study, the authors compared Dysport and Botox for the treatment of glabellar rhytids in 62 patients. The patients received either 20 total units of Botox or 50 total units of Dysport in their corrugator and procerus muscles and then underwent blinded assessments at maximal frown at 2, 8, 12, and 16 weeks after treatment. &lt;br /&gt;&lt;br /&gt;Fifty-nine patients completed the study; none dropped out because of side effects or lack of efficacy. Efficacy peaked at 8 weeks in both groups, but by 12 weeks the Botox group showed greater residual effects. By 16 weeks, this difference was statistically significant: On a four-point wrinkle-severity scale, a one-point or better improvement occurred in 53% of Botox recipients and 28% of Dysport recipients. At week 16, 23% of Botox patients and 40% of Dysport patients had recurrence of rhytids. Patient satisfaction was consistently higher with Botox. As in business, volume discounts can be found in medicine.&lt;br /&gt;&lt;br /&gt;Plastic surgery events known as Botox parties--also seminars, evenings and socials--are a key element of Botox marketing in much of the United States. The gatherings are thought to be a convenient means of providing Botox treatments more economically, and may help reduce the anxiety that normally goes along with getting an injection. Doctors are finding that treating people in groups allows them to make the procedure more affordable to their patients.&lt;br /&gt;&lt;br /&gt;Here's how a "party" typically works: A group of often nervous, but excited, middle-aged men and women mingle in a common area. Sometimes refreshments are served. One by one, as their name is called, each slips away for about 15 minutes to a private exam room. He or she pays a fee and signs an informed consent agreement. Anesthesia is rarely needed, but sedatives and numbing agents may be available. The practitioner injects about one-tenth of a teaspoon of toxin into specific muscles of the forehead most often targeted for the effect. The person then rejoins the group.&lt;br /&gt;&lt;br /&gt;Scott A. Greenberg, M.D., a board-certified plastic surgeon in Winter Park, Fla., has been hosting monthly "Botox Happy Hours" in his medical office since the drug's approval in April. Greenberg feels that these by-invitation-only events to previous patients "are an opportunity to treat a lot of people at one time in a relaxed but professional atmosphere." Greenberg says there is no difference between treating 10 people during individual office visits throughout the day and treating 10 people individually, but in a more socialized setting. "The important thing is that the identical standards of medical care are maintained at these gatherings as in a routine daytime office consultation."&lt;br /&gt;&lt;br /&gt;Julianne Clifford, Ph.D., of the FDA's Division of Vaccines and Related Products Applications, explains that "Botox is licensed for marketing and distribution as single-use vials." This means that as packaged, "each vial is intended to be used for a single patient in a single treatment session." Botox does not contain a preservative against potential contamination of the product through repeated use of a single vial. Once opened and diluted, Botox must be used within four hours. Treating multiple people with one vial violates product labeling, which is stated on the package insert, the vial and the carton.&lt;br /&gt;&lt;br /&gt;"We lose something when we mass treat," says Franklin L. DiSpaltro, M.D., president of the ASAPS. "One of my concerns is that these parties are a marketing tool--gathering as many patients as possible trivializes a medical treatment, which could deteriorate over time into a nonprofessional environment." DiSpaltro says there's more to medicine "than just dispensing drugs."&lt;br /&gt;&lt;br /&gt;Schwallenberg, however, insists that "Dr. Greenberg was very professional. It wasn't a cattle call," she says. "And I don't think I'd go to a doctor I didn't know."&lt;br /&gt;&lt;br /&gt;The FDA is concerned that Botox has the potential for being abused. The ASAPS recently reported that unqualified people are dispensing Botox in salons, gyms, hotel rooms, home-based offices, and other retail venues. In such cases, people run the risks of improper technique, inappropriate dosages, and unsanitary conditions. "Botox is a prescription drug that should be administered by a qualified physician in an appropriate medical setting," says Toombs.&lt;br /&gt;&lt;br /&gt;Greenberg agrees. "Patient safety has to be of prime concern," he says. "People need to be in the right hands when complications arise." That's why Greenberg does not allow his staff to administer Botox treatments. Even the most skilled health-care providers, he says, can have complications as well as dissatisfied customers.&lt;br /&gt;&lt;br /&gt;Although there is no chance of contracting botulism from Botox injections, there are some risks associated with the procedure. If too much toxin is injected, for example, or if it is injected into the wrong facial area, a person can end up with droopy eyelid muscles (ptosis) that could last for weeks. This particular complication was observed in clinical trials.&lt;br /&gt;&lt;br /&gt;Other common side effects following injection were headache, respiratory infection, flu syndrome, and nausea. Less frequent adverse reactions included pain in the face, redness at the injection site, and muscle weakness. These reactions were generally temporary, but could last several months.&lt;br /&gt;&lt;br /&gt;While the effects of Botox Cosmetic don't last, still, people don't seem to mind repeating the procedure every four to six months in order to maintain a wrinkle-free look. He first conceived a possible therapeutic use of botulinum toxin. In 1870, Muller (another German physician) coined the name botulism, from Latin botulus = "sausage". In 1895, Emile Van Ermengem first isolated the bacterium Clostridium botulinum. In 1944, Edward Schantz cultured Clostridium botulinum and isolated the toxin, and, in 1949, Burgen's group discovered that botulinum toxin blocks neuromuscular transmission.&lt;br /&gt;&lt;br /&gt;By 1973, Alan B Scott, MD, of Smith-Kettlewell Institute used botulinum toxin type A (BTX-A) in monkey experiments, and, in 1980, he officially used BTX-A for the first time in humans to treat strabismus. In December 1989, BTX-A (BOTOX) was approved by the US Food and Drug Administration (FDA) for the treatment of strabismus, blepharospasm, and hemifacial spasm in patients over 12 years old. The cosmetic effect of BTX-A was initially described by the Carruthers, a dermatologist/ophthalmologist husband and wife team working in Vancouver, Canada, although the effect had been observed by a number of independent groups. On April 15, 2002, the FDA announced the approval of botulinum toxin type A (BOTOX Cosmetic) to temporarily improve the appearance of moderate-to-severe frown lines between the eyebrows (glabellar lines). The drug previously was approved to treat two eye muscle disorders and a neurological movement disorder. &lt;br /&gt;&lt;br /&gt;FDA APPROVES BOTOX® TO TREAT FROWN LINES&lt;br /&gt;FDA today announced the approval of Botulinum Toxin Type A (BOTOX® Cosmetic) to temporarily improve the appearance of moderate to severe frown lines between the eyebrows (glabellar lines), a medical condition that is not serious. The product's manufacturer, Allergan, Inc., Irvine, California, is now allowed to market Botulinum Toxin Type A for this new indication. &lt;br /&gt;&lt;br /&gt;Botulinum Toxin Type A is a protein produced by the bacterium Clostridium botulinum. When used in medical settings as an injectable form of sterile, purified botulinum toxin, small doses of the toxin are injected into the affected muscles and block the release of the chemical acetylcholine that would otherwise signal the muscle to contract. The toxin thus paralyzes or weakens the injected muscle. &lt;br /&gt;&lt;br /&gt;BOTOX® was first approved in December 1989, to treat two eye muscle disorders (blepharospasm and strabismus) and in December 2000 to treat cervical dystonia, a neurological movement disorder causing severe neck and shoulder contractions.&lt;br /&gt;&lt;br /&gt;In placebo-controlled, multicenter, randomized clinical trials involving a total of 405 patients with moderate to severe glabellar lines who were injected with BOTOX® Cosmetic, data from both the investigators' and the patients' ratings of the improvement of the frown lines were evaluated. After 30 days, the great majority of investigators and patients rated frown lines as improved or nonexistent. Very few patients in the placebo group saw similar improvement. &lt;br /&gt;&lt;br /&gt;In these studies, the severity of the glabellar lines was reduced somewhat for up to 120 days for those patients who received BOTOX® Cosmetic. Most of the patients in the study were female, and the majority was under 50 years old. In large amounts, this toxin can cause botulism, which you probably associate with food poisoning. Despite the fact that one of the most serious complications of botulism is paralysis, scientists have discovered a way to use it to human advantage. Small, diluted amounts can be directly injected into specific muscles causing controlled weakening of the muscles. &lt;br /&gt;&lt;br /&gt;The FDA approved such usage in the late 1980s upon the discovery that Botox could stop ailments like blepharospasm (uncontrolled blinking) and strabismus (lazy eye). Cosmetic physicians have been using Botox for years to successfully treat wrinkles and facial creases. In April 2002, Botox gained FDA approval for treatment of moderate-to-severe frown lines between the eyebrows - called glabellar lines. However, Botox is often used for other areas of the face as well. &lt;br /&gt;&lt;br /&gt;How Does Botox Work?&lt;br /&gt;&lt;br /&gt;Botox blocks signals from the nerves to the muscles. The injected muscle can no longer contract, which causes the wrinkles to relax and soften.&lt;br /&gt;&lt;br /&gt;It is most often used on forehead lines, crow's feet (lines around the eye) and frown lines. As muscle action gradually returns, the lines and wrinkles begin to re-appear and wrinkles need to be re-treated. The lines and wrinkles often appear less severe with time because the muscles are being trained to relax.&lt;br /&gt;&lt;br /&gt;What Are the Side Effects of Botox?&lt;br /&gt;&lt;br /&gt;Temporary bruising is the most common side effect. Headaches, which resolve in 24-48 hours, can occur, but this is rare. A small percentage of patients may develop eyelid drooping. This usually resolves in three weeks. This development is usually caused by migration of the Botox and for this reason, you shouldn't rub the treated area for 12 hours after injection or lay down for three to four hours. There have been no allergies associated with Botox to date.&lt;br /&gt;&lt;br /&gt;Who Should Not Receive Botox Injection?&lt;br /&gt;&lt;br /&gt;Patients who are pregnant, breastfeeding or have a neurological disease should not use Botox.
